I need one of those signs

Like in factories. You know the ones that read:
No Accidents in ### Days
I need one that reads:
No Faulty QoD questions in ### Minutes
So far, I'm 1 for 3 with "clean" QoDs

IOW, today's has a problem. It doesn't help that I have to do my research at the office, where just about all the web is blocked.

I will say that I think there is a LOT of wrong information on the web on this one, but there's no way I can prove it is wrong.
