So even though every president elect since FDR -- and possibly going back as far as Washington -- has said it, I'm assuming Sarah Hughes was the first to actually prompt the president-elect to say "So help me God". Is that right?

The Washington story has a lot of doubters. According to the Library of Congress Chester Arthur is the first to have been confirmed to add it in 1881 when he was sworn in following the death of Garfield. Not every President since him has said it. Teddy Roosevelt in 1901 said "and thus I swear". I don't know if Sarah Hughes was the first to prompt it.

The question doesn't imply that President Johnson's swearing-in was unique, just unusual. Another oddity I didn't know until now: he put his left hand on President Kennedy's missal, not a Bible.

One thing I thought was really interesting was that Ms. Hughes came to Air Force One prepared to make up an oath, not realizing there was one in the Constitution.
